INSURANCE​

All international students enrolled in UPSI will be insured under Etiqa Takaful Insurance provider.

​

For your information, all government hospitals are Etiqa's panel hospital. International students who seek for medical treatment at any government hospital can apply for Guarantee Letter (GL) in order to admit into ward.

​

The following are the ward admission procedure into government hospital:-

​

Step 1: Get the Pre-admission/Pre-authorisation Form here.

​

Step 2: Ask the doctor or medical assistant whom referring the students/patients to fill up the form in case of ward admission.

​

Step 3: The complete form must be fax or emailed to 24-Hours EtiqaHealthCare Call Centre

               Fax no.: 1 800 229 988

Step 4: After the form has been faxed or emailed, verify the submission of the form with EtiqaHealthCare Call Centre through the hotline 1 800 889 998

 

Step 5: Wait for the feedback form the representative of EtiqaHealthCare Call Centre and  make a follow-up call (if any)

​

Step 6: GL will be endorsed if the treatment is covered OR a Decline Letter will be endorsed if the treatment is not covered under the Policy's Terms and Regulations. The GL will be faxed to the hospital's management

​

Step 7: The student can be admitted cashless into the hospital if it's covered or pay advance if the treatment is not covered or not allowed via cashless

​

For Outpatient treatment at a clinic or hospital, the student needs to pay in advance for the treatment first then need to submit the Original Receipt to the Student Affairs Division for reimbursement.
